What Are Goals of Care?

Goals of care are shared decisions made between you (the patient), your family or carers (if you choose), and your healthcare team. These discussions help clarify your preferences for treatment and care.

Why Are Goals of Care Important?

  • To make sure your healthcare reflects your values, beliefs, and wishes
  • To support clear communication between you and your care team
  • To help you understand your treatment options and outcomes
  • To ensure your care is respectful, coordinated, and planned for all stages of your journey

When Are Goals of Care Discussed?

  • On admission to hospital or during changes in your health care journey
  • Before major surgery or treatment
  • Daily with your care team

  • Your current health condition and expected progression
  • Your values, beliefs, and preferences

Who Is Involved?

  • You, the patient – your voice is central
  • Your family, carers, or nominated decision-maker, if you wish
  • Your treating doctors, nurses, and other healthcare professionals

Your choices are respected at all times. You can change your mind about your goals of care, and we will support you in reviewing them as needed.
