Prior to your admission with us, you will need to complete our admission forms. In order to ensure timely processing of your admission, we request that you complete the online admission form no later than 7 days prior to your admission.

Our online admission form is powered by Lifecard from Global Health. Global Health are the company who run our patient administration software. By completing your admission form online, you will be setting up a free account with them. The privacy and security of your personal health information is something we take extremely seriously and is our main priority. You can read Lifecard’s privacy policy here. If you do not wish to create an account, you have the option to fill in a hard copy admission form. Please ask your doctor or call our admissions team on 02 8624 5000 for more information.
